Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehabilitation services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement treatment plans, assist with therapeutic exercises and mobility training, and document patient progress. You'll collaborate closely with an interdisciplinary team, educate patients and families, and contribute to positive functional outcomes. Responsibilities Implement physical therapy treatment plans under supervision of a licensed Physical Therapist Assist patients with therapeutic exercises, gait training, and functional mobility activities Monitor, document, and report patient progress and response to treatment Educate patients, families, and caregivers on exercises, safety, and functional strategies Collaborate with an interdisciplinary team to support patient-centered care plans Maintain accurate, timely, and compliant clinical documentation Ensure safe use of equipment and adherence to infection control and safety protocols Support discharge planning with functional status updates and home exercise instruction Contribute to a respectful, dignified environment for seniors in skilled nursing and post-acute care Participate in continuing education, in-services, and professional development activities Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training and transfers Clinical documentation in EMR systems Knowledge of geriatric conditions and precautions Patient and family education Range-of-motion and strengthening techniques Safety, body mechanics, and fall prevention Vital signs monitoring and patient observation Interdisciplinary care coordination
