Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality rehab services to seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will deliver individualized treatment plans to improve mobility, function, and quality of life. Responsibilities Deliver PTA treatments under the direction of a Physical Therapist for senior patients in skilled nursing and post-acute care settings Implement individualized treatment plans to improve mobility, strength, balance, and functional independence Accurately document treatments, progress notes, and outcomes in the electronic health record Assist with patient assessments by providing clinical observations and feedback to the supervising PTEducate patients, families, and caregivers on home exercises, safety, and functional mobility strategies Collaborate with an interdisciplinary care team to support patient-centered goals and discharge planning Monitor patient response to treatment and communicate changes in condition promptly Contribute to a positive, respectful, and integrity-driven culture focused on quality outcomes Participate in mentorship, in-services, and ongoing professional development offered by Renewal Rehab Support departmental efficiency, including scheduling, caseload management, and compliance with regulatory standards Required Skills Therapeutic exercise implementation Gait and balance training Neuromuscular re-education Manual therapy techniques (within PTA scope) Transfer and mobility training Use of rehab modalities (e.g., e-stim, ultrasound) Electronic health record (EHR) documentation Care planning in SNF/post-acute settings Patient and family education Interdisciplinary team collaboration
