Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support delivery of high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working closely with supervising Physical Therapists, you'll implement individualized treatment plans, lead therapeutic exercises, document progress, and collaborate with an interdisciplinary care team. Our patient-centered, collaborative culture offers strong clinical resources, mentorship, manageable caseloads, and opportunities for professional growth while helping seniors improve function, safety, and quality of life. Responsibilities Implement physical therapy treatment plans under PT supervision Provide therapeutic exercises, gait and balance training for seniors Assist with functional mobility, transfer training, and fall prevention Monitor, document, and report patient progress and response to treatment Educate patients, families, and caregivers on exercises and safety Collaborate with PTs, nurses, and interdisciplinary team on care plans Ensure compliance with state, federal, and facility regulations Maintain accurate, timely therapy documentation and billing records Support discharge planning and recommendations for continued services Contribute to a positive, patient-centered senior care environment Required Skills Physical therapy interventions Therapeutic exercise instruction Gait and balance training Geriatric rehabilitation Functional mobility and transfer training Patient and family education Clinical documentation and EMR use Knowledge of SNF/post-acute regulations Safety and fall prevention Interdisciplinary team collaboration
