Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises, functional mobility, and balance training, and closely monitor patient progress. You'll document treatments, collaborate with an interdisciplinary team, educate patients and families, and contribute to a respectful, patient-centered environment focused on restoring function and improving quality of life. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait, balance, and functional mobility training Assist patients with safe use of assistive devices and transfer techniques Observe, measure, and document patient progress and response to treatment Communicate patient status and goals with PT and interdisciplinary team Educate patients and caregivers on home exercises and safety strategies Ensure accurate, timely clinical documentation per facility and regulatory standards Contribute to discharge planning and functional goal setting Maintain a safe, clean, and supportive treatment environment Participate in team meetings, in-services, and ongoing clinical education Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation techniques Skilled nursing/post-acute care rehab Patient transfer and safe handling Use of assistive devices Clinical documentation (EMR) Vital signs monitoring Patient and caregiver education
