Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide patient-centered care for seniors in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you will deliver therapeutic exercises, gait and balance training, and functional mobility interventions to help patients regain independence and improve quality of life. You'll collaborate closely with an interdisciplinary team, maintain accurate documentation, and support a culture grounded in integrity, respect, mentorship, and clinical excellence in senior care. Responsibilities Deliver therapeutic exercises, gait and balance training, and functional activities under the direction of a Physical Therapist Assist in evaluating patient progress and update treatment activities based on established care plans Accurately document treatments, patient responses, and progress in the EMR in a timely manner Collaborate with PTs, OTs, nursing, and facility staff to support comprehensive, patient-centered care Educate patients and families on exercises, safety, and strategies to maximize functional independence Adhere to all regulatory, ethical, and company standards, ensuring safety and infection control Contribute to a positive, collaborative team culture focused on quality clinical outcomes for seniors Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Patient assessment and documentation (EMR) Knowledge of senior/geriatric rehab care Modalities application (heat, cold, e-stim, etc.) Care planning and interdisciplinary collaboration Patient and family education Infection control and safety practices Time management and caseload prioritization
