Job Description

Renewal Rehab is seeking a caring Physical Therapist Assistant to provide hands-on rehab services for seniors in skilled nursing and post-acute settings. Working under the direction of a Physical Therapist, you'll deliver individualized treatments to improve strength, balance, mobility, and safety, while documenting progress in the EMR. You'll collaborate with an interdisciplinary team and educate patients and caregivers to support functional goals. Renewal Rehab offers a collaborative, patient-centered culture, strong clinical resources, mentorship, professional development, and manageable caseloads so you can focus on quality outcomes and meaningful daily impact. Responsibilities Deliver safe, evidence-based physical therapy interventions for seniors under the direction of a Physical Therapist Implement and progress treatment plans to improve functional mobility, strength, and balance Accurately document treatments, patient progress, and outcomes in the EMR in a timely manner Educate patients, families, and caregivers on exercises, safety, and functional strategies Collaborate closely with PTs, nurses, and facility staff to support patient-centered care plans Assist with discharge planning and recommendations for equipment or continued services Maintain compliance with facility, company, and regulatory standards and documentation requirements Contribute to a positive, respectful, and collaborative culture focused on quality outcomes Required Skills Therapeutic exercise implementation Gait training and balance interventions Manual therapy techniques (within PTA scope) Functional mobility and transfer training Use of therapy modalities (e.g., heat, cold, e-stim) Geriatric rehabilitation techniques Care plan implementation and progression under PT supervision EMR documentation and outcome tracking Patient and family education Interdisciplinary team collaboration
