Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support delivery of high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises and mobility training, document patient progress, and collaborate with an interdisciplinary team. Responsibilities Implement physical therapy treatment plans under PT supervision Assist with therapeutic exercises, gait, and balance training Monitor and document patient progress and response to treatment Educate patients and families on exercises and safety techniques Collaborate with PTs, nurses, and other team members on care plans Ensure accurate, timely, and compliant clinical documentation Maintain safe, clean treatment areas and follow infection control protocols Support safe patient handling and transfer techniques Participate in care conferences and interdisciplinary meetings Contribute to a positive, patient-centered rehab environment Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility training Geriatric rehabilitation Skilled nursing/post-acute care rehab Clinical documentation (EMR) Patient and family education Safe patient handling and transfers Vital sign monitoring and response Infection control and safety procedures
