Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to provide high-quality, evidence-based care for seniors in skilled nursing and post-acute settings. In this onsite role, you'll implement individualized treatment plans under the direction of a Physical Therapist, focusing on mobility, balance, strength, and safety to enhance functional independence and quality of life. You'll document in EMR, collaborate closely with an interdisciplinary team, and educate patients and families. Renewal Rehab offers a collaborative, patient-centered culture, strong clinical support, mentorship, manageable caseloads, and opportunities for professional growth as you make a daily impact on the seniors you serve. Responsibilities Deliver evidence-based physical therapy interventions under PT supervision for seniors in skilled nursing and post-acute care Implement individualized treatment plans focused on mobility, balance, strength, and functional independence Accurately document evaluations, daily notes, and progress in the EMR in a timely, compliant manner Monitor patient response, communicate changes to supervising PT, and adjust interventions within plan of care Educate patients, families, and caregivers on home programs, safety, and functional strategies Collaborate with an interdisciplinary care team to support optimal outcomes and discharge planning Contribute to a positive, patient-centered culture built on integrity, respect, and open communication Participate in mentorship, in-service training, and ongoing professional development opportunities Required Skills Therapeutic exercise implementation Gait training and balance retraining Functional mobility and transfer training Therapeutic modalities (e.g., heat, cold, e-stim) Documentation in EMR systems Care planning in skilled nursing/post-acute settings Patient and family education Vital signs monitoring and safety awareness Interdisciplinary team collaboration Knowledge of Medicare and rehab compliance
