Renewal Rehab seeks a compassionate Physical Therapist Assistant to provide high-quality rehab services to seniors in skilled nursing and post-acute settings. You will deliver treatments under a PT's direction, support individualized care plans, and document progress to improve mobility, function, and quality of life. Responsibilities Provide therapeutic exercises and functional training under the supervision of a Physical Therapist Implement individualized treatment plans for senior patients in skilled nursing and post-acute settings Accurately document treatments, patient responses, and progress in the medical record Collaborate with PTs, nursing, and interdisciplinary team members to optimize outcomes Educate patients, families, and caregivers on safe mobility and home exercise programs Assist with discharge planning and recommendations for continued services or equipment Adhere to all regulatory, ethical, and company standards of care and documentation Contribute to a respectful, patient-centered culture focused on quality outcomes Required Skills Therapeutic exercise techniques Gait training and balance intervention Functional mobility training (transfers, ADLs) Use of rehab equipment and modalities Geriatric and SNF rehabilitation practices Clinical documentation and EMR systems Patient and family education Fall prevention strategies Care planning and interdisciplinary collaboration Knowledge of Medicare and rehab-related regulations
