Renewal Rehab seeks a compassionate Physical Therapist Assistant to provide high-quality care for seniors in skilled nursing and post-acute settings. Working within an interdisciplinary team, you'll implement treatment plans designed by Physical Therapists, support safe mobility, and help patients improve strength, balance, and functional independence. You'll document progress, educate patients and families, and collaborate with staff to ensure excellent clinical outcomes. Responsibilities Provide therapeutic exercises and interventions under the direction of a Physical Therapist Assist with gait training, balance, strength, and transfer training for senior patients Implement treatment plans and adjust interventions within scope based on patient response Accurately document treatments, patient progress, and outcomes in the medical record Educate patients, families, and caregivers on safe mobility and home exercise programs Collaborate with an interdisciplinary care team to support discharge planning and goals of care Ensure safety and proper use of equipment, following infection control and facility protocols Participate in care conferences and communicate patient status to supervising PTs and nursing staff Support quality improvement, outcome tracking, and evidence-based practice initiatives Uphold Renewal Rehab's values of integrity, respect, and patient-centered care in all interactions Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility training Geriatric rehabilitation Skilled nursing/post-acute care documentation Patient and family education Use of assistive devices and adaptive equipment Range of motion and strengthening techniques Pain management modalities within PTA scope Interdisciplinary care coordination
