Job Description
Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high-quality rehabilitation services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises and mobility training, and document progress to maximize functional outcomes and quality of life. You'll collaborate closely with an interdisciplinary team, maintain strong communication with patients and families, and uphold our culture of integrity, respect, and patient-centered care while benefiting from mentorship and professional development. Responsibilities Implement physical therapy treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, balance and transfer training for senior patients Assist with functional mobility and activities of daily living to improve independence Accurately document treatments, patient responses, and progress in the medical record Communicate effectively with PTs, nursing staff, physicians, patients, and families Monitor patient safety and adhere to all infection control and facility policies Educate patients and caregivers on home exercises and mobility strategies Participate in interdisciplinary care conferences and discharge planning Contribute to quality improvement and outcome tracking initiatives Uphold Renewal Rehab's values of integrity, respect, and patient-centered care Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility training Geriatric rehabilitation techniques Skilled nursing/post-acute care documentation Use of assistive devices and adaptive equipment Patient and caregiver education Electronic medical record (EMR) documentation Infection control practices Interdisciplinary team collaboration
