Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support therapy services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, guide therapeutic exercises, and document progress to enhance functional outcomes and quality of life. Responsibilities Implement physical therapy treatment plans under the supervision of a Physical Therapist Provide therapeutic exercises, gait training, and functional mobility interventions for senior patients Accurately document patient treatments, progress, and responses in accordance with regulatory standards Assist in patient assessments by reporting observations and changes in functional status to the supervising PTEducate patients, families, and caregivers on home exercise programs and safety strategies Collaborate with an interdisciplinary care team to support holistic patient care and discharge planning Maintain compliance with facility, state, and federal regulations and company policies Contribute to a positive, respectful, and patient-centered therapy environment Participate in ongoing professional development and in-service training Support efficient caseload management while prioritizing quality outcomes Required Skills Therapeutic exercise implementation Gait training and balance activities Functional mobility and transfer training Geriatric rehabilitation techniques Safe patient handling and body mechanics Clinical documentation and EMR use Knowledge of long-term care and post-acute regulations Patient and family education Interdisciplinary care coordination Infection control and safety practices
