Working under the direction of a licensed Physical Therapist, you will carry out treatment plans, lead therapeutic exercises, document patient progress, and collaborate with an interdisciplinary team. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Assist patients with therapeutic exercises, gait training, and functional mobility Observe, measure, and document patient progress and response to treatment Report clinical changes and collaborate with PT and interdisciplinary care team Educate patients, families, and caregivers on exercises and safety techniques Maintain accurate and timely clinical documentation per regulatory standards Ensure safe use of equipment and adherence to infection control protocols Contribute to care planning, discharge planning, and goal setting Support a patient-centered environment focused on functional outcomes Participate in ongoing training, mentorship, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait training and balance interventions Functional mobility training (transfers, bed mobility) Geriatric and post-acute rehabilitation techniques Vital sign monitoring and patient observation Clinical documentation and EMR use Patient and caregiver education Infection control and safety procedures Therapeutic modalities (e.g., heat, cold, e-stim as allowed by state law) Interdisciplinary team collaboration and communication
