Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our Healthcare & Medical Services team serving seniors in skilled nursing and post-acute care. In this on-site role, you will provide hands-on treatments under the direction of a Physical Therapist, focusing on mobility, strength, balance, and safe functional independence. You'll document progress, collaborate with an interdisciplinary team, and educate patients and families. Renewal Rehab offers a supportive, patient-centered culture, clinical mentorship, manageable caseloads, and opportunities to grow while making a daily impact on seniors' quality of life. Responsibilities Deliver skilled PTA treatments under the direction of a licensed Physical Therapist for seniors in post-acute and long-term care settings Implement individualized treatment plans to improve mobility, strength, balance, and functional independence Document treatments, progress notes, and outcomes accurately and timely in the EMREducate patients, families, and caregivers on exercises, safety, and functional strategies to support carryover Collaborate with PTs, OTs, nursing, and facility staff to coordinate care and optimize outcomes Participate in care conferences, screenings, and reassessments as directed by the PTMaintain compliance with state practice guidelines, company policies, and payer requirements Contribute to a positive, respectful, and patient-centered therapy environment Required Skills Therapeutic exercise implementation Gait training and balance interventions Manual therapy and functional mobility training Treatment planning under PT supervision Geriatric rehabilitation techniques Patient and family education Clinical documentation and EMR use Vital signs monitoring and basic safety assessment Interdisciplinary team collaboration Knowledge of Medicare and rehab documentation standards
