Job Description
Renewal Rehab seeks a compassionate Physical Therapist Assistant to support high-quality rehab services for seniors in skilled nursing and post-acute care settings. Working under the direction of a Physical Therapist, you will implement treatment plans, assist with therapeutic exercises and mobility training, document progress, and educate patients and families. You'll collaborate with an interdisciplinary team in a patient-centered, integrity-driven culture that offers mentorship, strong clinical resources, and opportunities for professional growth while positively impacting patients' functional outcomes and quality of life. Responsibilities Implement PT treatment plans under PT supervision Assist with therapeutic exercises, gait, and balance training Monitor, document, and report patient progress and response to treatment Support safe patient transfers and functional mobility training Educate patients, families, and caregivers on exercises and safety Collaborate with PTs, nurses, and interdisciplinary team members Maintain accurate, timely, and compliant clinical documentation Adhere to ethical, regulatory, and company quality standards Participate in care planning and discharge planning as requested Contribute to a positive, respectful, and patient-centered environment Required Skills Therapeutic exercise implementation Gait and balance training Functional mobility and transfer training Geriatric rehabilitation techniques Skilled nursing/post-acute care documentation Use of rehab equipment and modalities (per scope) Patient and family education Interdisciplinary team collaboration Electronic medical record (EMR) documentation Knowledge of Medicare and payer regulations for rehab
