Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support high quality rehabilitation services for seniors in skilled nursing and post acute care settings. Working under the direction of a Physical Therapist, you will implement individualized treatment plans, guide therapeutic exercises, monitor patient progress, and document care accurately. You'll collaborate closely with an interdisciplinary team, educate patients and families, and help promote safety and functional independence. Responsibilities Implement PT treatment plans under supervision of a licensed Physical Therapist Provide therapeutic exercises, gait training, and functional mobility interventions Assist with balance, strength, and endurance training for senior patients Monitor, document, and report patient progress and response to treatment Educate patients and families on home exercises, safety, and functional strategies Collaborate with an interdisciplinary team to support care planning and discharge goals Maintain accurate, timely, and compliant clinical documentation Ensure safe use of equipment and adherence to infection control and safety standards Participate in case conferences, in services, and clinical training/mentorship Contribute to a positive, patient centered culture focused on quality outcomes Required Skills Therapeutic exercise implementation Gait training and transfer techniques Functional mobility and balance training Patient and family education Geriatric rehabilitation techniques Clinical documentation and EMR use Vital signs monitoring and safety awareness Range of motion and strengthening programs Knowledge of PT modalities and equipment Interdisciplinary team collaboration
