Renewal Rehab is seeking a compassionate Physical Therapist Assistant to support seniors in skilled nursing and post-acute care settings. Under the direction of a Physical Therapist, you will implement individualized treatment plans, assist with therapeutic exercises and mobility training, and document patient progress. You'll collaborate with an interdisciplinary team to promote safety, function, and quality of life. Responsibilities Implement treatment plans under the supervision of a Physical Therapist Assist patients with therapeutic exercises, gait, and balance training Monitor and document patient progress and response to treatment Educate patients and caregivers on home exercises and safety Collaborate with PTs, nurses, and interdisciplinary team members Ensure compliance with regulatory, documentation, and billing standards Maintain a safe, clean, and patient-centered treatment environment Contribute to discharge planning and functional goal setting Participate in in-services, training, and professional development Uphold company values of integrity, respect, and quality outcomes in senior care Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility assistance Geriatric rehabilitation Skilled nursing/post-acute care familiarity Clinical documentation and EMR use Patient and caregiver education Safety and fall prevention Range of motion and strengthening techniques Vital signs monitoring Functional mobility assessment Interdisciplinary team collaboration
