Job Description

Renewal Rehab is seeking a compassionate Physical Therapist Assistant to join our on-site rehab team serving primarily seniors in skilled nursing and post-acute care. In this role, you will implement individualized physical therapy treatment plans under the direction of a Physical Therapist, focusing on restoring mobility, function, and independence. You'll provide hands-on therapeutic exercises, gait and balance training, and patient education while accurately documenting outcomes. Responsibilities Provide physical therapy interventions under the direction of a licensed Physical Therapist. Deliver individualized treatment sessions focused on mobility, strength, balance, and functional independence. Treat primarily senior patients in skilled nursing and post-acute care settings. Accurately document treatments, patient progress, and outcomes in the EMR system. Educate patients and families on exercises, safety, and strategies to support recovery. Collaborate with an interdisciplinary team, including PTs, OTs, nursing, and physicians. Assist with care planning and progress reviews to optimize functional outcomes. Follow all clinical, regulatory, and documentation standards and guidelines. Contribute to a positive, patient-centered, and team-oriented work culture. Participate in mentorship, in-service training, and continuing education offered by Renewal Rehab.
