Our primary areas include Buffalo, Delano, Cokato, Rockford, Watertown, Waverly, Maple Lake, and surrounding communities. Additional patient coverage extends toward the Mound/Long Lake/Medina area and west toward Annandale, Kimball, South Haven, and Cold Spring . Other communities within our broader service area include Corcoran, Greenfield, Hamel, Howard Lake, Independence, Loretto, Maple Plain, Montrose, and Hanover . The goal is not to have one PTA driving through all 23 cities. Patient schedules are built around geography, patient needs, and efficient territory coverage. Turn the PT's Plan Into Progress You'll work closely with the supervising Physical Therapist to carry out individualized plans of care. Your treatment visits may include strengthening, gait and balance training, transfers, mobility, functional exercises, home exercise programs, and patient or caregiver education. Just as importantly, you'll see how the patient responds between visits. You'll communicate changes and progress back to the PT so treatment can continue evolving with the patient. What You'll Be Doing Provide skilled Physical Therapy treatment in patient homes Implement plans of care established by the supervising PT Provide strengthening, gait, balance, transfer, and mobility training Reinforce individualized home exercise programs Educate patients and caregivers on safe mobility and functional techniques Monitor patient response and progress between visits Identify and communicate meaningful clinical changes to the supervising PT Complete accurate and timely EMR documentation Collaborate with PTs, nurses, OTs, and other interdisciplinary clinicians Your PTA Experience Can Transfer to Home Health You don't need to have spent your entire career in home health. PTAs coming from Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Senior Living, Long-Term Care, Orthopedics, and other rehabilitation settings can bring highly transferable skills to this role.
