But you'll also be paying attention. How is the patient responding? Is mobility improving? Are they becoming more confident? Has their fall risk changed? Is the home exercise program working? Is there a new limitation the PT needs to know about? Good PTAs don't simply complete the treatment plan. They observe what happens while they're carrying it out. What You'll Be Doing Provide skilled Physical Therapy treatment in patient homes Implement individualized plans of care established by the supervising PT Provide strengthening, gait, balance, transfer, mobility, and functional training Reinforce individualized home exercise programs Educate patients and caregivers on safe mobility Monitor response to treatment and functional progress Recognize and communicate meaningful changes to the supervising Physical Therapist Support fall prevention and home safety goals Complete accurate and timely EMR documentation Collaborate with PTs, OTs, nurses, and other interdisciplinary clinicians Bring Your Rehab Experience Into Home Health You don't need previous home health experience to bring valuable skills to this role. PTAs from Outpatient Therapy, Skilled Nursing, Inpatient Rehabilitation, Transitional Care, Orthopedics, Senior Living, Long-Term Care, Hospital Rehab, and other rehabilitation settings can transition well into home-based care. We're looking for someone who connects naturally with patients, provides strong hands-on treatment, communicates well with the supervising PT, and can confidently manage a field-based workday. What You Bring Active Minnesota Physical Therapist Assistant (PTA) license Valid driver's license, reliable transportation, and automobile insurance Strong hands-on treatment skills Excellent patient and caregiver communication Strong observation and clinical communication skills Ability to independently manage a field-based schedule Strong organization and documentation skills Home health experience and experience with geriatric or medically complex patients are preferred, but not required.
