Job Summary We are seeking a dedicated and compassionate Physical Therapist Assistant to join our healthcare team. The ideal candidate will provide comprehensive therapy services to diverse patient populations, including pediatrics, geriatrics, and individuals with disabilities. This role involves developing personalized care plans, assisting with skilled nursing facilities. The therapist will collaborate with multidisciplinary teams to ensure optimal patient outcomes and quality of care. Duties Assess patients' physical, cognitive, and emotional needs to develop individualized treatment plans. Assist patients with activities of daily living (ADLs) to promote independence and improve quality of life. Implement therapeutic interventions tailored to specific populations based on setting Coordinate discharge planning and case management to ensure seamless transitions across care settings. Work with patients in skilled nursing facilities. Collaborate with healthcare professionals to monitor progress and adjust treatment plans accordingly. Educate patients and families on therapy techniques, adaptive equipment use, and safety measures. Maintain accurate documentation of patient assessments, treatment plans, progress notes, and discharge summaries in compliance with regulatory standards. Stay current with advancements in occupational therapy practices and participate in ongoing professional development. Qualifications Valid state licensure or certification as a Physical Therapist Assistant. Experience working with diverse populations. Knowledge of ADLs assistance, discharge planning, case management, physiology, anatomy, medical terminology, and patient care protocols. Strong communication skills and ability to work collaboratively within multidisciplinary teams. Ability to adapt treatment approaches based on individual patient needs and care settings.
