Job Description

Tender Touch Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide high-quality therapy services to seniors in skilled nursing and long-term care settings, implementing treatment plans under the direction of a Physical Therapist. You will help improve residents' function, independence, and quality of life while collaborating with an interdisciplinary team. Responsibilities Deliver individualized treatment plans under the direction of a Physical Therapist for seniors in skilled nursing and long-term care settings. Provide therapeutic exercises, gait and balance training, and functional mobility interventions to maximize safety and independence. Accurately document treatments, progress, and patient responses in the electronic medical record in a timely manner. Collaborate with PTs, OTs, speech therapists, nursing, and physicians to support coordinated, patient-centered care. Educate patients, families, and caregivers on home exercise programs, fall prevention, and safe mobility strategies. Assist with screenings, reassessments, and discharge planning to support appropriate level of care and continuity. Uphold dignity, compassion, and respect in every interaction, reflecting Tender Touch Rehab Services' values. Participate in in-services, mentorship, and continuing education to support clinical growth and best practices. Required Skills Therapeutic exercise implementation Gait training and balance interventions Neuromuscular re-education Manual therapy techniques (basic PTA level) Transfer and mobility training Use of therapeutic modalities (e.g., heat, cold, e-stim per state law) Functional assessment and documentation (EMR) Care planning in skilled nursing/LTC settings Patient and family education Interdisciplinary team collaboration
