Job Description

Tender Touch Rehab Services is seeking a dedicated Physical Therapist Assistant to join our Healthcare & Medical Services team. In this role, you will provide high-quality therapy services to seniors in skilled nursing and long-term care settings, implementing treatment plans under the direction of a Physical Therapist. You will help improve residents' function, independence, and quality of life while collaborating with an interdisciplinary team. Responsibilities Deliver skilled PTA treatments under the direction of a Physical Therapist to senior patients in skilled nursing and long-term care settings Implement and modify individualized treatment plans to improve mobility, function, and independence Provide gait training, strengthening, balance, and functional mobility interventions with a focus on safety and fall prevention Accurately document treatments, patient progress, and outcomes in the electronic medical record in a timely manner Educate patients, families, and caregivers on home exercises, safe mobility, and equipment use Collaborate closely with PTs, OTs, SLPs, nursing, and medical staff to support coordinated, patient-centered care Participate in care conferences and contribute to discharge planning and goal setting Uphold regulatory, ethical, and company standards while promoting dignity, respect, and compassion for all patients Engage in ongoing professional development, mentorship, and clinical training offered by Tender Touch Rehab Services Required Skills Therapeutic exercise implementation Gait and balance training Neuromuscular re-education Functional mobility and transfer training Use of therapeutic modalities (heat, cold, e-stim) Documentation in EMR systems Care plan implementation and progression Patient and family education Safety and fall prevention Interdisciplinary team collaboration
