Job Description

Tender Touch Rehab Services seeks a compassionate Physical Therapist Assistant to support high quality rehab services for seniors in skilled nursing and long term care. Partnering with Physical Therapists, you'll carry out individualized treatment plans, lead therapeutic exercises, gait and balance training, and functional mobility activities to improve safety, independence, and quality of life. Collaborate closely with an interdisciplinary team, document progress accurately, educate patients and families, and contribute to a supportive, patient centered culture that values dignity, respect, professional growth, and evidence based practice. Responsibilities Implement physical therapy treatment plans under PT supervision for geriatric patients in skilled nursing and long-term care settings Provide therapeutic exercises, gait training, balance and transfer training to improve function and safety Assist with functional mobility, ADL-focused interventions, and fall-prevention strategies Observe, measure, and document patient progress, response to treatment, and functional outcomes in EMR systems Collaborate with PTs, nurses, physicians, and interdisciplinary team members to coordinate patient-centered care Educate patients, families, and caregivers on home programs, safety, and strategies to maintain functional gains Adhere to all regulatory, documentation, and billing requirements for rehab services Maintain a safe, clean treatment environment and ensure proper use of equipment and assistive devices Contribute to care planning, discharge recommendations, and goal setting based on patient needs and progress Participate in professional development, mentorship, and quality improvement initiatives Required Skills Therapeutic exercise implementation Gait and balance training Transfer and mobility training Geriatric rehabilitation Skilled nursing/long-term care rehab Functional outcome measurement and documentation Electronic medical record (EMR) documentation Patient and family education Safe patient handling and body mechanics Knowledge of rehab regulations and billing compliance
