The PTA provides superior customer service by modeling the Unity Brand Promise and Core Values.

SCHEDULE
Day shift, 8-hr. shifts (8:00 am•4:30 pm).
Weekend requirement:
one weekend shift per month
RESPONSIBILITIES
Within standards of practice established for a PTA, completes and documents (within agency standards) related to patient care, including visit notes, renewal orders, and verbal orders. Includes progress or lack of goal achievement in all charting. Implements treatment programs individualized to meet patient needs and discharge plans: issues and provides instruction in the use of adaptive equipment. Confers with therapist directly, physician, and/or interdisciplinary team members regarding patient response to therapy and any concerns. Provides PT gait and ambulation training. Instructs in the use of assistive devices. Provides instruction in safe transfer using appropriate body mechanics and equipment. Participates in discharge planning and provides timely and adequate patient and family teaching to facilitate the discharge plan. Coordinates patient schedule with team members and readily and effectively adjusts the schedule to meet changes in census and patient needs.
REQUIRED QUALIFICATIONS
Associate's degree from an accredited Physical Therapy Assistant program. Reads, writes, and speaks English Must demonstrate computer proficiency. Excellent interpersonal and communication skills. Required Licensure /
Certification Skills:
Current NYS PT Assistant licensure required

PHYSICAL REQUIREMENTS
M•Medium Work•Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. Any physical requirements reported by a prospective employee and/or employee's physician or delegate will be considered for accommodations.
