Overview Homestead Health Care Center is a post-hospital rehabilitation and skilled nursing care facility that offers comprehensive Post-Acute Recovery programs in a quaint, home like atmosphere. Our staff is specifically trained to care for and manage the needs of the residents while working together with a team of physicians, nursing staff, therapists, and dieticians.. Responsibilities Implement prescribed physical therapy treatments under the supervision of licensed therapists, ensuring adherence to care plans and clinical protocols. Assist with clinical assessment documentation by recording patient progress, responses to therapy, and any changes in condition using electronic health records (EHR). Operate medical equipment safely and effectively during therapy sessions, including manual therapy tools and physiotherapy devices. Communicate clearly with patients and caregivers to explain treatment procedures, answer questions, and provide encouragement throughout the rehabilitation process. Collaborate closely with healthcare professionals such as physicians, occupational therapists, and nurses to coordinate patient care plans and discharge strategies. Maintain accurate medical documentation in compliance with clinical standards and regulatory requirements. Support discharge planning by preparing reports on patient progress and recommending ongoing care needs for post-treatment follow-up. Skills Strong knowledge of physiology, anatomy, orthopedics, geriatrics, pediatrics, and related medical terminology essential for effective treatment implementation. Proficiency in clinical assessment documentation standards and medical documentation practices within electronic health records (EHR). Experience with skilled nursing facilities is highly desirable. Skilled in manual therapy techniques such as trigger point therapy and other physiotherapy modalities to alleviate pain and improve mobility. Excellent caregiver and client communication skills to foster trust and motivate patients through their recovery journey. Ability to operate medical equipment safely while adhering to safety protocols. Familiarity with discharge planning processes and interdisciplinary collaboration for comprehensive patient care.
