$5,000 Sign On Bonus Schedule Monday -

Friday:

8:30 AM - 5:00 PM No weekends or on-call requirements Territory North Broadway, Cleveland Agency Southwest General Home Health What We Offer Guaranteed base hourly rate + uncapped bonus potential Mileage reimbursement Additional bonus opportunities Flexible scheduling with dedicated scheduling/admissions support Medical, dental & vision insurance Paid Time Off (PTO) 401(k) Company-paid life insurance Employee Assistance Program (EAP) Qualifications Current Physical Therapist Assistant (PTA) license in the state of employment 1+ year of PTA experience in an acute care setting required Home health experience preferred Strong communication and clinical decision-making skills Knowledge of physical therapy best practices Valid driver's license, reliable transportation, and auto insurance Key Responsibilities Provide skilled physical therapy services in patients' homes under the direction of a Physical Therapist Follow and implement individualized treatment plans Monitor, document, and report patient progress and changes Educate patients and caregivers on therapy techniques, safety, and equipment use Collaborate with physicians, therapists, and interdisciplinary care teams Complete accurate, timely documentation in compliance with Medicare, federal, and agency guidelines Participate in case conferences, training, and quality improvement activities Requirements Travel throughout the assigned territory Ability to lift up to 50 lbs.
