Physical Therapy Assistant (PTA)

Buckeye Home Health Care is looking for a per diem Physical Therapy Assistant to join our team. This position would offer a stable caseload and will cover the Green County area. We are looking for someone who can help improve the well being of our patients and their families and has the heart and skills to work with home bound patients. The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee.

Qualifications:

Provide care wherever the patient calls home. Oversee the PT plan of care as directed by the patient's physician and PT. Work with other disciplines to give the best care needed for each patient. Ability to complete documentations within an EMR system Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed. When appropriate teach and counsel family members about care and processes for disease/injuries. Attend meetings, CEUs, sometimes in the Dayton office/area

Requirements:

Computer skills; must be able to work within EMR system. Must have current PTA Ohio license in good standing. Reliable auto and current auto insurance. This position requires traveling . Current CPR. Can work well independently • New graduates are welcome to apply.
