Skilled Nursing Facility (SNF) Make a Difference in Recovery and Independence Join our rehabilitation team as a Physical Therapist Assistant (PTA) providing services within a Skilled Nursing Facility in Daingerfield, Texas . In this role, you'll work alongside a licensed Physical Therapist to help patients regain strength, mobility, function, and independence following illness, injury, surgery, or changes in physical ability. This is an excellent opportunity for a compassionate PTA who enjoys building relationships with patients and seeing their progress firsthand. What You'll Do Carry out individualized treatment plans established by the supervising Physical Therapist Provide therapeutic exercises, functional mobility training, manual techniques, and approved treatment modalities Assist patients with improving strength, balance, coordination, mobility, and overall physical function Monitor patient response to treatment and communicate progress, concerns, or changes to the supervising Physical Therapist Accurately document treatments and patient progress according to company and regulatory guidelines Educate patients and families on exercises, assistive devices, safety, and strategies to promote continued independence Collaborate with the interdisciplinary care team to support positive patient outcomes Maintain a safe, organized, and clean treatment environment Qualifications Graduate of an accredited Physical Therapist Assistant program Current and valid Physical Therapist Assistant (PTA) license in the state of Texas Current AHA CPR certification Valid driver's license Valid auto insurance Previous rehabilitation, skilled nursing, or home health experience preferred HomeCare HomeBase (HCHB) experience preferred Strong communication, documentation, and patient-care skills Physical Requirements Ability to stand, walk, sit, climb, crawl, squat, and kneel for extended periods Ability to lift, carry, pull, or push up to 100 lbs. as required to safely assist patients 401(k) Benefits Eligible after 3 months of service . 100% immediately vested Choice Health at Home contributes 100% of the first 3% you contribute each pay period Bring Your Skills to a Team That Makes an Impact If you're a licensed PTA who is passionate about helping patients regain mobility, confidence, and independence, we'd love to hear from you.
