Job Description
Capstone of Hughes Springs full-time PT or PTA Position Summary Capstone of Hughes Springs is seeking a full-time Physical Therapist (PT) or Physical Therapist Assistant (PTA) to join our home health team. In this role, you will help patients improve mobility, relieve pain, build strength, restore physical function, and manage disabling conditions caused by illness or injury. The ideal candidate is compassionate, dependable, and committed to providing high-quality, patient-centered care in the home setting. Key Responsibilities Physical Therapist Perform and document initial evaluations. Review clinical information to identify functional limitations and establish a physical therapy diagnosis. Develop individualized plans of care designed to improve or restore physical function, alleviate pain, and prevent further impairment. Provide skilled physical therapy interventions in accordance with each patient's plan of care. Evaluate patient progress at appropriate intervals and modify treatment plans to achieve the greatest possible benefit. Provide appropriate supervision and direction to Physical Therapist Assistants, when applicable. Physical Therapist Assistant Provide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist. Follow the established plan of care and report patient progress, concerns, and changes in condition to the supervising Physical Therapist. Document treatments, patient responses, and other required clinical information accurately and promptly. Assist patients in improving mobility, strength, balance, function, and safety. All Candidates Follow infection-control standards and universal precautions while providing patient care. Promote continuity and quality of care through accurate, timely documentation. Communicate effectively with patients, families, caregivers, team members, and other healthcare professionals. Educate patients and caregivers regarding treatment plans, therapeutic activities, safety, and home exercise programs. Follow company policies, administrative procedures, and standards of professional practice. Complete all required documentation within company-established timeframes. Travel to patients' homes throughout the assigned service area. Qualifications Successful completion of an accredited Physical Therapist or Physical Therapist Assistant program, as applicable. Current, valid state license as a Physical Therapist or Physical Therapist Assistant. Current CPR certification. Valid driver's license and automobile insurance. At least one year of home health experience preferred. Homecare Homebase experience preferred. Strong communication, organization, and time-management skills. Physical Requirements Candidates must be able to: Stand, walk, sit, climb, crawl, squat, and kneel for extended periods. Lift, carry, pull, or push up to 100 pounds. Perform the physical activities necessary to provide safe patient care in a home environment.
