Responsibilities Conduct comprehensive assessments of patients' physical conditions, including those with orthopedic, neurological & geriatric population. Develop and implement individualized treatment plans that promote recovery, mobility, and functional independence. Perform therapeutic interventions such as manual therapy, balance training, ther ex, transfer training, gait training, and patient education to optimize rehabilitation outcomes. Manage clinical documentation standards accurately within electronic health records (EHR) systems, ensuring compliance with healthcare regulations. Collaborate effectively with physicians, nurses, caregivers, OT, SLP and other healthcare professionals to coordinate care plans and discharge planning strategies. Provide expert guidance on physiotherapy techniques and physiology knowledge to support patient rehabilitation. Engage in patient communication to motivate and educate clients about their treatment plans while addressing individual concerns and progress updates. Qualifications Demonstrated experience to treat in home health environment with prior home health experience preferred. Strong understanding of medical terminology, anatomy knowledge, physiology principles, and clinical documentation standards. Experience working with geriatric patients population is highly desirable. Knowledge of discharge planning processes and working with care plans across diverse healthcare environments such as orthopedics, neurology, geriatrics. Proficiency in electronic medical records (EMR) systems Kinnser and HCHB for documentation management. Ability to collaborate seamlessly with multidisciplinary teams to develop treatment plans that meet patient goals. Excellent client communication skills combined with a compassionate approach to home & community care delivery

Qualifications & Requirements Education:

Doctor of Physical Therapy (DPT) or a Master's degree from an accredited physical therapy program. Active unencumbered Physical Therapist Assistant (PTA) license in the state of Texas.

Licensure:

An active, unrestricted PT/PTA license in the State of Texas.

Certifications:

Current Basic Life Support (BLS) / CPR certification.

Experience:

Minimum of 1 year of clinical PT experience required; prior home health experience is strongly preferred.

Travel:

A valid driver's license, reliable personal vehicle, and active automotive insurance to navigate the Tarrant county coverage areas.
