Job Description

We are seeking an experienced, licensed Physical Therapist Assistant to join the physical therapy department within our Sherman pain management practice. The PTA will deliver skilled physical therapy interventions under the supervision of, and in accordance with plans of care established by, our Lead Physical Therapist. This is a focused outpatient role. Our PT clinic operates inside the pain management practice and treats our own patients exclusively — there is no external referral development, no community marketing, and no need to build a caseload. Patients arrive already established with our physicians, typically with imaging, diagnostics, and treatment history. This role suits an experienced, self-directed PTA. You will be the primary therapy provider in the clinic on most of your scheduled days, responsible for both patient care and the day-to-day operation of the therapy space. This is not a role for a new graduate or for someone who prefers a heavily staffed, closely-shadowed environment. About the Setting Our patient population is primarily musculoskeletal and chronic pain. You can expect to work with: Post-injection and post-procedure patients following interventional pain treatments. Chronic low back, neck, and radicular pain. Degenerative joint and spine conditions. Persistent regional pain, myofascial pain, and deconditioning secondary to chronic pain. Workers' compensation and personal injury cases with associated documentation requirements. Patients working toward functional restoration, activity tolerance, and reduced reliance on passive care. Because the PT clinic sits inside the physician practice, communication with treating providers is direct and same-day. A minimum of one Medical Doctor and two Nurse Practitioners are on site every day the clinic operates, so medical questions, patient concerns, and changes in status can be addressed immediately without waiting on outside coordination. Supervision Structure The supervising Physical Therapist establishes and modifies all plans of care, performs all evaluations and re-evaluations, and remains professionally responsible for the physical therapy services you provide. The Lead PT is available by phone at all times during clinic treatment hours, on every scheduled day, and can be reached immediately for questions about patient status, treatment progression, or any concern arising during a session. Video consultation is available when a situation calls for it — so the PT can directly observe a movement pattern, review technique, or assess a patient's presentation in real time rather than relying on verbal description alone. The Lead PT is currently on site one scheduled day per week for evaluations, re- assessments, plan-of-care review, and documented PT/PTA conferences. The on-site physicians and nurse practitioners are available for medical concerns; physician presence does not substitute for physical therapy supervision, which is provided exclusively by the supervising PT. Key Responsibilities Patient Care Deliver skilled therapeutic interventions in accordance with the plan of care established by the supervising Physical Therapist. Implement therapeutic exercise, functional mobility training, manual therapy techniques within PTA scope, gait training, and neuromuscular re-education as assigned. Apply physical agents and modalities as directed within the plan of care. Monitor and document patient response, including pain behavior, functional tolerance, and progress toward established goals. Recognize and promptly report changes in patient status, adverse responses, or red- flag findings to the supervising PT. Progress or regress assigned interventions within the parameters set by the supervising PT. Escalate acute medical concerns to the on-site medical team while notifying the supervising PT. Chronic Pain-Specific Care Reinforce pain neuroscience education and self-management strategies as directed by the PT. Support graded activity, activity pacing, and graded exposure approaches for patients with fear-avoidance behaviors. Coach patients through flare-ups and setbacks with an emphasis on function over symptom elimination. Maintain a consistent, encouraging approach with patients who may have long treatment histories and prior unsuccessful care episodes. Workers' Compensation & Personal Injury Maintain documentation meeting Texas Division of Workers' Compensation standards, including objective functional measures and clear treatment justification. Track visit counts and authorized treatment against approved parameters; alert the supervising PT and administrative staff before authorizations lapse. Document with the understanding that records may be reviewed by adjusters, designated doctors, peer reviewers, or counsel. Support work-status and functional-capacity communication as directed by the supervising PT and treating physician. Maintain the same objective, consistent documentation standard across all payer types. Documentation & Communication Complete accurate, timely daily treatment notes consistent with practice standards and payer requirements. Participate in documented conferences with the supervising PT regarding patient status and progression, at the frequency the PT determines. Contact the supervising PT by phone during the treatment day whenever a patient is not progressing as expected, has an adverse response, or presents anything that may warrant plan-of-care modification — the PT is reachable at all times and expects to be called rather than updated after the fact. Communicate relevant clinical observations to the on-site PM&R physician as appropriate. Support accurate billing practices, including appropriate use of required modifiers for services furnished by a PTA. Clinic Operations This clinic currently does not employ a physical therapy aide or technician. The PTA is responsible for the operation of the therapy space on scheduled days: Prepare, turn over, and clean treatment areas between patients. Set up, break down, and maintain therapeutic equipment and modality units. Maintain therapy supply inventory. Manage daily schedule flow and patient throughput. Perform routine equipment checks; report maintenance and calibration needs. Maintain a clean, safe, organized, and compliant clinic environment. Coordinate with front office staff on scheduling, check-in, and authorization status. Scope of Practice Consistent with Texas Board of Physical Therapy Examiners rules, the PTA in this role does not: Perform re-evaluations and discharge assessments. Interpret, or modify the plan of care. Perform patient assessment procedures requiring the judgment of a PT. Recommend assistive devices, orthoses, prostheses, or architectural modifications. Sign documentation that designs or modifies the plan of care. Supervise the treatment of patients receiving physical therapy services. Required Qualifications Graduate of a CAPTE-accredited Physical Therapist Assistant program. Current Texas PTA license in good standing Minimum 2 years of post-licensure PTA experience — outpatient orthopedic, pain, or musculoskeletal preferred. Current BLS/CPR certification. Demonstrated ability to work independently within an established plan of care, with the judgment to know when to hold, modify within parameters, or escalate. Proven competence in therapeutic exercise, functional mobility training, manual. techniques within scope, and modality application. Working proficiency with electronic medical records (ECW) and same-day documentation discipline. Reliable availability for the agreed 3-4 day weekly schedule. Preferred Qualifications Experience with chronic pain, functional restoration, or post-interventional populations. Workers' compensation and/or personal injury documentation experience. Familiarity with pain neuroscience education, graded activity, or motivational interviewing approaches. Experience in a physician-integrated or multidisciplinary practice setting. Experience as the therapy provider in a clinic or satellite location. Manual therapy coursework within PTA scope. Bilingual English/Spanish preferred but not required. Attributes We're Looking For Self-sufficiency — comfortable running the clinic day independently and managing your own time. Sound clinical judgment about when to advance, hold, or escalate without waiting for direction. Proactive communicator who picks up the phone in the moment rather than saving questions. Patience and consistency with patients whose progress is measured over months, not visits. Comfort working within another clinician's plan of care while contributing meaningful observation. Willing to do the unglamorous work-patient turnover, and equipment upkeep are part of this job. Growth Opportunity This position is structured at 2-4 days per week with a defined path to full-time as clinic volume grows. We are looking for someone who wants to build a long-term role here, not fill a gap. As the department expands, the PTA in this position will have first consideration for expanded hours and for input into future support staffing. Physical Requirements Ability to stand, walk, kneel, squat, and move about the clinic for the majority of a shift Ability to lift, transfer, guard, and physically assist patients, including those requiring substantial support. Ability to lift and move equipment and supplies up to thirty (30) pounds. Manual dexterity sufficient for manual therapy techniques and equipment operation. Ability to demonstrate therapeutic exercises and functional movement patterns.
Schedule & Compensation Schedule:
2-4 days per week, Monday-Thursday, approximately 32 hours weekly. Clinic hours are 8-5. No Friday-Sunday, no call.
Compensation:
