Functions or Scope of Assigned Duties:

Assesses and documents medical history

  • diagnosis
  • and plan of care Initiates
  • orders
  • and evaluates plans of care for a panel of outpatient clients Provides follow-up exams for mediation management and monitoring Provides patient/family education regarding disease process
  • risk factor
  • symptom control
  • and treatment (including use of medication) Consults with other providers and multidisciplinary team
