Job Description

LifeCare Community Health Center is a Federally Qualified Health Center (FQHC) with three locations in Stark County, Ohio. Eligible providers may qualify for student loan repayment opportunities through the National Health Service Corps (NHSC). Position Description Position Title:
Practice Registered Nurse (APRN) or Certified Physician Assistant (PA-C)

Position Summary The Advanced Practice Provider serves as a primary healthcare provider within the individual's applicable scope of practice, licensure, certification, and organizational clinical guidelines. The provider works collaboratively with other members of the healthcare team to provide comprehensive, high-quality care to LifeCare patients. Essential Duties and Responsibilities Provides comprehensive, patient-centered healthcare within the provider's scope of practice. Performs patient assessments, physical examinations, preventive health services, screenings, and other appropriate clinical procedures. Diagnoses and treats health conditions within the provider's scope of practice and refers patients for specialty or additional care when appropriate. Coordinates patient services and referrals to promote continuity of care. Provides patient education and counseling regarding diagnoses, treatment plans, preventive care, and health-related concerns. Provides care and support for patients with chronic health conditions and collaborates with other healthcare professionals as appropriate. Prescribes medications and medical devices in accordance with applicable Ohio law, licensure, credentials, and scope of practice. Maintains accurate, complete, and timely documentation in the electronic health record (EHR). Works collaboratively and professionally with physicians, advanced practice providers, clinical staff, support staff, and other members of the healthcare team. Demonstrates effective communication and interpersonal skills with patients, families, coworkers, and other healthcare professionals. Maintains flexibility with patient scheduling and adapts to changing patient-care and clinic needs. Participates in quality and performance improvement activities. Maintains required continuing education, professional credentials, and clinical competencies. Complies with LifeCare policies and procedures and applicable HIPAA, OSHA, and other federal and state requirements. Assists with clinic, outreach, and organizational activities as needed. Travels when necessary to meet operational needs. Performs other duties as assigned. Minimum Qualifications Current Ohio licensure as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA), as applicable to the individual's profession. Current applicable national certification. Appropriate prescriptive authority and DEA registration, as required for the position. Current BLS certification or ability to obtain within the timeframe required by LifeCare. Ability to practice within applicable federal and Ohio laws and professional licensing requirements. Knowledge, Skills and Abilities Strong interpersonal and communication skills. Ability to work effectively and collaboratively as a member of a healthcare team. Ability to establish and maintain positive professional working relationships. Ability to work effectively with a culturally diverse patient population. Knowledge of clinical procedures and standards appropriate to the position. Ability to use electronic health records and other computer-based systems. Ability to adapt to changing patient-care needs and priorities. Must be flexible with patient scheduling and hours based on patient and operational needs. Professional, dependable, and team-oriented.
