Job Description
Seeking a Primary Care Physician for a senior-focused program serving individuals 55+ with chronic illnesses or disabilities to promote independence in their homes and communities. Responsibilities include delivering comprehensive primary care, developing individualized care plans with an interdisciplinary team, managing chronic and acute conditions, providing preventive services, and participating in care coordination and quality improvement efforts. Qualifications include MD/DO with interest in geriatrics, board certification or eligibility, state license, DEA registration, and at least one year of experience working with the elderly.
