Family Medicine Physician -
Santa Cruz County, CA Clinical Role:
Permanent position with flexible 4-day work week Full-time and part-time options available 100% outpatient family medicine with opportunity for dedicated pediatrics time if desired See 20 patients per day Team-based medical home model with on-site dental, optometry, behavioral health, lactation support, parent educators, diabetic nurse educator, and MAT program
EMR:
Greenway Intergy Due to patient population, must be bilingual in Spanish Not able to sponsor visa candidates
Compensation & Benefits:
Salary:
$270,400 - $304,337.58 annually (DOE & FTE)
Sign-on/Retention Bonus:
Up to $30,000 (FTE prorated) 50% paid on first paycheck, 25% after year 1, 25% after year 2 Relocation assistance: Up to $10,000 Benefits include: Medical, dental, vision, life insurance 19 PTO days + 12 paid holidays annually 401(k) with employer contribution CME allowance Reimbursement of medical license fees Voluntary long-term disability State Loan Repayment Program (SLRP) eligibility
