2996667722-2 Apply for this job now Job Description Hartford Healthcare seeks an Endocrinology Advanced Practice Provider to deliver high-quality, patient-centered care for adults with diabetes and complex endocrine disorders. In this role, you will perform comprehensive assessments, interpret diagnostic tests, and manage treatment plans including insulin and advanced technologies. Working within our collaborative, multidisciplinary team, you'll provide in-person and telehealth visits, patient education, and chronic disease management. Responsibilities Provide comprehensive endocrine evaluations and ongoing management for patients with diabetes, thyroid, pituitary, adrenal, and metabolic disorders. Conduct detailed histories, physical exams, interpret lab and imaging results, and formulate evidence-based treatment plans. Initiate and adjust insulin and other endocrine medication regimens, including use of insulin pumps and continuous glucose monitoring systems. Educate patients and families on disease management, lifestyle modification, and self-management techniques to optimize outcomes. Collaborate closely with endocrinologists, primary care providers, diabetes educators, and multidisciplinary teams to coordinate care. Perform follow-up visits (in-person and virtual), monitor treatment response, and modify plans based on clinical guidelines and patient goals. Document all encounters, orders, and care plans accurately in the electronic health record in compliance with regulatory and institutional standards. Participate in quality improvement, clinical protocols, and population health initiatives within the endocrinology service line. Precept students or new APPs as needed and engage in ongoing professional development and specialty education.
