Job Description
Position Summary A well-established, not-for-profit community hospital is seeking a Primary Care Physician (MD/DO) to join its outpatient practice in Oxford, Connecticut . Candidates with a background in Family Medicine or Internal Medicine will be considered. This is a full-time, permanent outpatient position providing comprehensive primary care services to patients across all age groups. The role focuses on preventive care, chronic disease management, and continuity of care in a patient-centered environment. Both experienced physicians and recent graduates are encouraged to apply. Essential Responsibilities Provide comprehensive primary care services, including preventive care, chronic disease management, and acute visits Diagnose, treat, and manage a wide range of medical conditions across all age groups Perform annual physicals, wellness visits, and routine health screenings Order, review, and interpret diagnostic tests and laboratory results Develop and implement individualized treatment plans for patients Collaborate with specialists and interdisciplinary care teams for coordinated patient care Maintain accurate and timely documentation within the EMR system Participate in quality improvement initiatives and evidence-based care delivery Educate patients on disease prevention, treatment plans, and healthy lifestyle choices
