Job Overview We are seeking a dedicated and compassionate Primary Care Physician D.O. or M.D. to join our dynamic healthcare team. Responsibilities Conduct thorough medical history reviews and physical examinations to assess patient health status. Diagnose and treat common illnesses, injuries, and chronic conditions across various age groups. Develop and implement individualized care plans, including preventive care and health education initiatives. Utilize Electronic Health Record (EHR) systems to document patient encounters accurately and efficiently. Collaborate with healthcare professionals such as specialists, nurses, pharmacists, and therapists to coordinate patient care. Order and interpret diagnostic tests including laboratory work, medical imaging, and other relevant assessments. Provide guidance on lifestyle modifications, medication management, and wellness strategies to promote overall health. Experience Experience with Advantage Medicare such as Humana Gold Plus Medical degree (D.O. ,M.D.)from an accredited institution with valid licensure to practice medicine in the designated region. Board Certified Family Medicine Post-residency experience in primary care or family medicine demonstrating comprehensive patient management skills. Proficiency in using EMR/EHR systems such as Eclinical or similar platforms for documentation and patient tracking. Experience with outpatient settings, clinic-based care. Knowledge of healthcare diagnosis procedures across specialties such as cardiology, pulmonology, endocrinology, dermatology, pediatrics, neurology, psychiatry, and others listed in required skills. Familiarity with diagnostic radiology procedures like CT scans or medical imaging interpretation is advantageous. Ability to perform basic life support (BLS) certification and demonstrate excellent patient service skills. Strong communication skills with a collaborative approach to working with multidisciplinary teams.
