Job Description
Primary Care Physician - Geriatrics - Lake Mary, FL Make a meaningful impact in the lives of senior patients while practicing quality-focused primary care. Provenir Healthcare is working with an established healthcare organization in Lake Mary, Florida , seeking a Board-Certified or Board-Eligible Internal Medicine or Family Medicine Physician to join its outpatient team. This is a great opportunity for a physician who enjoys building long-term patient relationships and practicing in a collaborative, value-based environment. The practice is dedicated to caring for seniors while giving physicians the opportunity to focus on preventive care, chronic disease management, wellness, and quality outcomes . Position Highlights 100% Outpatient Primary Care Monday-Friday, 8:00 AM-5:00 PM Average 15 patients per day Primarily Senior/Medicare Advantage population Value-based care model focused on quality over volume Collaborative, team-oriented environment eClinicalWorks (eCW) EMR Geriatric Fellowship graduates welcome Responsibilities Provide comprehensive primary care to adult and senior patients Evaluate and manage acute and chronic medical conditions Provide preventive care and wellness services Develop individualized treatment plans and manage medications Build lasting relationships with patients through continuity of care Collaborate with a team focused on value-based care and improved outcomes Qualifications MD or DO Board Certified or Board Eligible in Internal Medicine or Family Medicine Active, unrestricted Florida medical license Value-based care experience preferred Geriatric Fellowship graduates welcome Interested in learning more?
