Upto $290K salary + up to 20% of base salary in bonus†Competitive sign on bonus Bonus†based on: 1. Patient satisfaction rating 2. Living the company†s values 3. HEDIS compliance 4. Reducing hospital sick days Primary Care â€" Outpatient Low patient volume - 15-18 patients per day Value Based Care Model Patient population: Seniors with 3-5 complex diseases (Hearth disease, diabetes, hypertension etc.)
