  $310,000+ guaranteed base salary, plus wRVU productivity upside with low threshold

• Degree:

  MD or DO

• Certifications:

  Board Certified or Board Eligible in Family Medicine

• Licensure:

  Active Kentucky license or eligible to obtain one
