Job Description
Seeking a full-time Physician passionate about team collaboration, community-based care, and serving elderly and disabled populations. Functions as part of an interdisciplinary team to develop and monitor comprehensive care plans, providing geriatric and medical expertise. Supervises nurse practitioners or clinical nurse specialists, offering preventive, episodic, and chronic care, including physical and mental assessments. Evaluates members, orders tests, interprets results, manages acute and chronic conditions, and adjusts medications accordingly. Educates patients and families on health maintenance and illness management. Requires MD degree, MI license, DEA registration, board certification or eligibility in Family, Internal Medicine, or Geriatrics with at least 1 year of primary care experience with at-risk or elderly populations.
Schedule:
