Aptos; mso-hansi-theme-font:minor-latin; mso-font-kerning:1.0pt; mso-ligatures:standardcontextual;} We are seeking a Board-Certified Site Physician to join our team in Brooklyn, New York and provide comprehensive medical care to patients at a Community Health/Day Treatment Center. This position offers a consistent Monday-Friday schedule and the opportunity to make a meaningful impact caring for a frail and elderly patient population. The Site Physician will oversee the professional, clinical, organizational, and administrative aspects of patient care and work closely with an interdisciplinary team to coordinate high-quality, patient-centered services. Compensation & Benefits Base salary $240,000-$280,000 depending on experience, plus bonus Comprehensive benefits package Full-time Monday-Friday schedule, 8:00 AM-5:00 PM Combination of site-based and home-care services Collaborative interdisciplinary practice environment
Requirements:
Graduate of an accredited medical school. Completion of Internal Medicine or Family Medicine residency. Board Certified in Internal Medicine or Family Medicine required. Geriatric Medicine certification, fellowship, or experience preferred. Minimum one year of experience caring for frail or elderly patients preferred. Home-care experience preferred; must be willing to provide home-care services. Current, unrestricted New York State medical license. Active DEA license. Current CAQH profile. Active Medicare and Medicaid provider IDs. Excellent clinical, communication, organizational, and interpersonal skills. No malpractice or loss-of-license history related to practice at the DTC.
