Job Description

The Complex Care Clinic Physician delivers high-quality, person-centered, team-based primary care to a panel of 400+ medically complex, high-risk patients. This role emphasizes value-based care, population health management, chronic disease management, and care coordination across the care continuum. Care is provided in the outpatient setting via in office, home, and virtual visits. The selected physician will become a member of Summa Health Medical Group, a multi-specialty group practice of nearly 700 providers employed by Summa Health. Summa Health - headquartered in Akron, Ohio - is one of the largest integrated Healthcare Delivery Systems in the state. With 2 hospitals and 20+ health centers, Summa Health provides access to care throughout the region. Position Highlights Full-time (1.0 FTE), Monday through Friday, No Weekends 100% Outpatient; 6 to 8 Patients/Day Primary Location is 75 Arch Street, Akron, OH with minimal requirement to go to satellite facilities Robust interdisciplinary team support Strong connections with community resources Provide longitudinal primary care for patients with complex medical, behavioral, and social needs Minimum Qualifications Board Certified in Family Medicine, Internal Medicine, or Internal Medicine-Pediatrics Active and unrestricted Ohio medical license Minimum 3+ years post-residency clinical experience in primary care, hospital medicine, or complex care management Excellent communication and patient engagement skills, including working with caregivers and families Interested candidates should send CV to physicianrecruitment@summahealth.org or call Emily Lloyd, Provider Recruiter at 330-375-3155.
