Declare Wellness Center is an independent functional medicine practice founded by Dr. Craig Cohen, DC, FNP, and Amanda Cohen in Murrells Inlet, South Carolina. We are a growing, independent, cash-pay functional medicine and wellness practice in Murrells Inlet, South Carolina, specializing in bioidentical hormone replacement therapy, hormone optimization, metabolic health, genetics, and personalized wellness care.

ABOUT THIS POSITION
Declare Wellness Center is looking for an experienced Nurse Practitioner or Physician Assistant who wants to practice medicine differently. Here, you'll have the time and support to build meaningful relationships with patients, understand what's actually happening with their health, and help guide them toward measurable improvements in how they feel and function. Why This Position Is Different We believe there is a better way to practice medicine. Our patients come to us because they want a proactive, personalized approach to their health. They are invested in understanding their bodies, improving their health, and actively participating in the process. Instead of managing a high-volume schedule of disconnected visits, you'll build long-term relationships with patients and follow their progress through labs, hormone optimization, nutrition, lifestyle interventions, genetics, and other individualized therapies. You'll also work closely with our clinical founder, health coaches, and support team rather than being expected to figure everything out on your own. For a provider who loves learning, connecting the dots, and helping people become healthier, this can be an incredibly rewarding way to practice. Specialized Clinical Training Provided Previous functional medicine or BHRT experience is welcomed, but it is not required. We are willing to train the right experienced clinician who is curious, coachable, and genuinely interested in health optimization. You'll receive structured mentorship and hands-on training in areas including:
  • Bioidentical hormone replacement therapy and hormone optimization
  • Functional and preventative medicine
  • Advanced laboratory interpretation
  • Metabolic health and body composition
  • Genetics and personalized health recommendations
  • Peptide therapy
  • Nutrition, supplementation, and lifestyle interventions Our goal isn't simply to teach you protocols.
We want to help you develop the clinical reasoning and confidence to understand why you're making recommendations and how to individualize care for the person sitting in front of you. For the right provider, this is an opportunity to develop a highly specialized clinical skill set while being paid to practice it. 4.
KEY RESPONSIBILITIES
As part of our clinical team, you'll progressively develop your own patient relationships while working closely with our founder and clinical support team.
Your responsibilities will include:
  • Conducting patient consultations, review-of-findings appointments, and follow-up visits
  • Managing BHRT and hormone optimization care
  • Reviewing and interpreting laboratory results
  • Developing and adjusting individualized treatment plans
  • Ordering appropriate follow-up laboratory testing
  • Prescribing and managing therapies within your scope of practice
  • Collaborating with our health coaches to coordinate medical and lifestyle interventions
  • Communicating with patients between visits when clinical questions arise
  • Participating in clinical case reviews and team training
  • Maintaining timely and accurate clinical documentation
  • Helping educate patients through occasional workshops and educational sessions You'll be given time, training, and support to learn the Declare model before being expected to manage patients independently.

SKILLS & QUALIFICATIONS
Required Active Nurse Practitioner or Physician Assistant license South Carolina licensure or eligibility to obtain SC licensure before starting Prescriptive authority Minimum of 2 years of post-licensure clinical experience Strong patient communication and interpersonal skills Comfortable using EMR systems, laboratory platforms, and other digital tools Preferred but not required BHRT or hormone optimization experience Functional medicine experience or certification Experience with metabolic or weight-management medicine Peptide therapy experience Genetics or nutrigenomics experience Cash-pay or membership-based healthcare experience Experience in primary care, internal medicine, family medicine, women's health, endocrinology, cardiology, urgent care, weight management, or similar clinical environments can all translate well to this position. 6.

COMPENSATION & BENEFITS
Base Salary:
$110,000
  • 130,000, depending on experience
Performance Bonus:
Eligible after successful completion of the 90-day introductory period

We want to be transparent about this early in the process. Team members may obtain coverage through a spouse or partner, the ACA marketplace, or another individual coverage option. Build Your Career Near the South Carolina Coast Declare Wellness Center is located in Murrells Inlet, just south of Myrtle Beach. For candidates considering a move, the area offers coastal living, beaches, boating, golf, restaurants, outdoor recreation, and a growing community while maintaining a more relaxed lifestyle than many larger metropolitan areas. Relocation candidates are welcome. 8.
WHAT SUCCESS LOOKS LIKE HERE
During your first 90 days, you'll work closely with our team learning the Declare clinical model, developing your functional medicine and hormone knowledge, and progressively taking responsibility for patient follow-ups and lab reviews. By six months, our goal is for you to confidently manage your own patient relationships, collaborate seamlessly with our health coaches and clinical team, and feel comfortable making individualized recommendations within our model of care. But the most important measure of success is simpler: Your patients trust you, feel heard by you, and are making meaningful progress because of the care you provide. 9.
