Conviva Senior Primary Care seeks a Physician Assistant to deliver compassionate primary care to adults 65+. In this role, you'll perform evaluations, manage chronic conditions, and coordinate preventive and wellness-focused care in a value-based model. You'll collaborate closely with physicians, nurses, and care teams to create personalized care plans that support independence and quality of life.
