100 Hresan Boulevard Fayetteville, WV 25840 PRN Attending Physician (15-20 Hours/Month) The Personalized Health Partners medical practice is currently searching for a Primary Care Physician to provide care at our Fayetteville & Beckley, WV practices.

100 Heartland Dr, Beckley, WV 25801 As Primary Care Physician, you will be part of an exciting new model of care combining good old fashioned bedside manner with utilizing innovative technologies to dramatically reduce patients' return to hospital and achieve better outcomes.
Job Duties and Responsibilities :
Assume the administrative authority, responsibility, and accountability of implementing our medical services, policies, and procedures according to federal, state and local regulations; and current practice of post-acute and long-term care medicine. Coordinate medical care, including emergency treatment procedures, maintain effective liaison with attending physicians, and implement methods to keep the quality of care under constant surveillance. Provide necessary physician services for those residents within skilled nursing facilities and/or clinic environment
Qualifications :
M.D. or D.O. required Gerontology certification preferred MD or DO license in State of Ohio ABMS Board Certified in: Internal Medicine, Family Practice, Geriatrics or Hospice/ Palliative 5+ years of experience working with geriatric or long-term care population Demonstrated experience working with and educating
NP's Preferred Qualifications :
Experience in LTC setting preferred Familiarity with CMS, Medicare Advantage, SNP Rules and Regulations HCC and Risk Adjusted reimbursement methodology Inpatient/SNF utilization metrics and bed day management State specific collaborative practice agreements and midlevel practitioner licensing Knowledge of EMR
