May be available based on facility needs Requirements Current and valid LPN/LVN license applicable to Wisconsin. Previous experience in Long-Term Care, Skilled Nursing, or a similar setting preferred. Ability to provide safe and compassionate care to residents. Strong clinical, communication, and organizational skills. Ability to administer medications and treatments accurately. Knowledge of nursing procedures, infection-control practices, and patient safety standards. Ability to work effectively both independently and as part of a healthcare team. Must be reliable and able to work the assigned evening schedule. Must meet all applicable credentialing, compliance, and onboarding requirements. Responsibilities Provide direct nursing care to residents in accordance with established care plans. Administer medications and treatments as ordered. Monitor residents' conditions and report changes promptly to the appropriate healthcare provider. Perform assessments and maintain accurate and timely clinical documentation. Assist with activities of daily living and promote resident comfort and dignity. Follow individualized care plans and nursing protocols. Monitor vital signs and other relevant clinical indicators. Communicate effectively with physicians, nurses, CNAs, residents, and families. Maintain infection-control and patient-safety standards. Respond appropriately to changes in residents' conditions and emergency situations. Participate in care planning and interdisciplinary team activities as required. Maintain confidentiality and protect residents' rights and privacy. Perform other nursing duties within the scope of practice as assigned.
