Responsibilities:

Conduct thorough assessments of residents' needs and develop individualized care plans in collaboration with the interdisciplinary team Evaluate and revise care plans as necessary, ensuring alignment with regulatory requirements and individual resident needs Coordinate and facilitate care planning meetings with the interdisciplinary team, including family members and caregivers Develop and maintain accurate and detailed medical records, ensuring compliance with regulatory standards Provide guidance and support to nursing staff on resident care and care planning Collaborate with other departments, such as social services and therapy, to ensure comprehensive care
